When in installing a double 20lb metal base how many bolts do you put into the concrete. The base has 12 holes in it, 6 front and 6 back. Do you need to use all holes, are 4 front, 4 back enough? Does base need to be grouted?

The more the better but really its only a 20lbs washer. Not that big a deal. Usually there are 2 front 2 middle and 2 rear. Not Knowing where yours are located I would just use the ones in the 4 corners. Grout also.
If you have to level with washers as spacers then do so at the base to floor rather than base to machine.

I have a Double base (1 base for 2 20lb washers) and their are 6 bolts across the front and 6 across the rear, none in the middle.

I have three 18lb on one base and used six bolts total. No problems in over a year.

One of the problems in installing bases is when you drill the hole. A concrete drill with a impact driver can easily blow out the bottom of the hole being drilled. Start with a smaller bit and then progress to larger bits. The epoxies [glue] work better than compression anchors. A little extra time on the install will save you from a lot of head-aches later. If you use washers use LARGE ones. Paul

What kind of epoxy? I am trying anchoring cement it dried
very quick. I will tighten bolts tommorrow. I could not find
epoxy made for anchoring.

Don't take any short cuts. A minimum would be the same # and same approximate location as the machine's bolt holes. (line up the base bolts in the same approximate locations as the machine mounting bolts.) A small vibration in the steel base will become greater as it transfers up the sides of the machine. As Mark said, level the base. As far as grouting goes, If you decide to grout, leave a few openings where water can drain out. Check the slope in the floor.

A successful way to grout , which i learned from my experienced (40+yrs !) installer, is to actually 'sink in' the base into a 'mud' of concrete - thereby making the undersupport very strong and the base vibration-free.
When i say sink, i dont mean bury, just that the concrete rises a bit on the inside of the steel frame.
As to whether to grout at all, or not - i couldn't conceive of a steel base being of any use WITHOUT grouting. You'd be asking for a lot of vibration problems.
BTW, i've only used 4 bolt-holes per machine on both 25 & 30 lb machines. No untightening issues.

I checked my machines installation today and found that all my machines (from 20 to 60 lbs capacity) are on four bolts only. Two in the front and two in the back at each corner. I suspect my installer forgot to put 2 bolts in the middle across the machine. I am upset now because I don't know what the consequence will be.

Are they on metal bases or concrete pad?

Most installers don't use the center holes, too much efoort on there part.

The washers are bolted to the concrete pad.
Yes, Marc. I can see it is hard to reach the 2 holes in the center.

I have the steel bases and they have 6 bolts holding them into the floor for every 2 machines and are grouted. I also noticed that the bases are about halfway filled with sand. Who ever installed them must have done this to absorb the vibration.

Mylaundry: That is the case w/mine, as well as several other mats i've observed. Installers have told me they typically don't place the center bolts, unless it is really large (.i.e. over 50 lbs)

Do you think that the manufacturers went through all the trouble of designing, engineering, and machineing a machine with 6 bolt holes if it only needed 4 ????
If you only got 4 bolts and not 6, Then you probably got short changed on the installation. We have seen Unimac 50's shaking because one of the center bolts had broken or become loose !!!
